Cicero Health Dept offers Covid-19 Booster vaccines and 3rd doses
 
Please use the following link to make an appointment:
 
 
Select the vaccination you would like, Moderna or Pfizer, and then use one of the following Registration Codes when you complete the Registration Form.
 
FOR MODERNA: modernab1101
FOR PFIZER: pfizerb1101
 
(All appointments will begin Monday Nov. 1, 2021)
