RENEWALS: If the passport was issued prior to your sixteenth birth date, you will need to apply as you did the first time. If you were 16 years and older when the passport was issued, you may complete a DS-82 form (a blue & white form) with a personal check or money order of $130.00 made payable to the U.S. Department of State.
